Before your first appointment, please complete the form shown below. You can download and print out our form using the following links.
Dr. Randall Moody is a provider for Medicare. Provider status for other insurance carriers is pending. We provide insurance billing for primary and secondary insurance. Anything not covered by insurance will be your responsibility. We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may require you to pay a co-payment. This is due at the time of your appointment. Credit cards are welcome. Questions regarding your coverage and benefits should be directed to your employer or insurance company.
Dr. Moody will see patients in his office exclusively. He does not provide hospital coverage. Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
